How to redeem:

1. Purchase a Ti32, TiR32, Ti29, TiR29, Ti27, TiR27 Thermal Imager between September 1 and February 29, 2012.
2. Complete each field in the contact information. Print legibly. If the information on this form is not legible your rebate request will be invalid.
3. Note the purchase date and select the value of your rebate that matches your purchase.
4. Select your rebate amount carefully, if the cash rebate does not match the product purchased your rebate request may be invalid.
5. Mail the completed form with a copy of your original sales invoice to the address below. Distributors’ invoices must clearly validate “date of order” or if this information isn’t included on the invoice, then Customer is solely responsible to include “copy of original PO” in the rebate submission package.

6. One form can be used for multiple unit purchases but the model and number of units must be noted on the rebate form and complete supporting sales documents outlined in # 5 above must be included with your rebate form.

Responses postmarked more than 30 days after date of purchase or received more than 45 days after purchase will be invalid and will not be returned.
